Request for Replacement Parchment

Instructions - Please read before proceeding
How to apply for a replacement parchment:
Complete the below payment form and submit. A Tax Invoice will then be emailed to you.
Please print this receipt.
Complete the
Request for Replacement Parchment Form (RP Form)
including the statutory declaration on the last page.
Submit your Replacement Parchment form, payment receipt and supporting documentation to the address listed on the replacement parchment form:

Requests for replacement parchments
take approximately ten (10) working days to process
after receipt of
all
required documentation.
Your replacement parchment will be sent to your current mailing address as provided by you in section 1.
Domestic Items will be sent via eParcel registered mail and will require a signature upon delivery.
International mail can be sent via DHL Courier at an additional cost (see payment details section). All other international mail will be sent standard airmail and cannot be tracked.

3. REASON FOR REQUEST AND REQUIRED DOCUMENTATION
Reason for request (tick whichever is applicable)
Original has been lost or misplaced. A Statutory Declaration witnessed by a Justice of the Peace or overseas equivalent must accompany your request.
Original has been damaged. Your damaged original parchment must be mailed to the address listed at the top of the Request for Replacement Parchment form.
Change of name. Certified copies of supporting documentation must accompany your request. Your parchment will be issued as per your personal details provided in the Replacement Form.
Digital Copy required for employment, visa or proof of qualification.

4. PAYMENT DETAILS

Please provide a Street Address and local phone number in the above details for DHL courier delivery.
If you would like your replacement parchment to be mailed to an overseas address but do not select international courier, we will send your request via standard airmail. Please note standard airmail deliveries cannot be tracked.
